Supposedly, one of my ancestors was "dismissed" by the church for marrying
outside the faith. He was later granted a certificate to Third Haven MM, but
there's no information whether his wife joined the church or what the reason
was for being granted the certificate. It may not have been a full
reinstatement either.
I imagine that for members who had lived in the same place where everyone knew
them, asking permission from the elders was merely a formality. In other
circumstances where someone had just moved into the community, there was
probably some process of investigation.
