query page like Marlys', please forward me this info:

Your name (first name if that is your preference)
Your county(s) and state(s)
Your range of years for the query
Limitations
Color scheme preference for your page

The way it works, when someone fills in the blanks and hits "Submit", an
email is generated to you with the query info. You will then receive the
query by email to which you can respond in you own way.
