You can go to the LDS catalog anf download the form for ordering photocopies of films.
You do need to also go through the catalog and figure out which film this cert is on.
Fill out the form and mail or fax it to Salt Lake City and you will then receive the cert in the mail in about 2-4 weeks. The copy is usually of excellent quality.
The cost is $4 for the first item, $2 for each additional for a total of 8 items ($16) and this can be done every 2 weeks.
