Is anyone on the list familiar with an "Undertakers Death Certificate"
issued in Chicago cir 1907?

I have one of these for a possible ancestor. The certificate says across
the top " Department of Health:City of Chicago". Underneath that it says
"Undertakers Report of Death". It has much of the same info as a normal death
certificate, but it is missing a place for spouses name, home address, parents
names, Informant (other than the funeral home). It does have an area for
physicians cause of death and where the decedent died.

Has anyone seen one of these? Should there also be a separate official
death certificate? The number on this "undertakers report of death" was obtained
from the state of Illinois pre 1916 death index online.
