Thanks Cheri, you have given me much to think about and also solved my
problem regarding my file number. I now know how to proceed. Regarding the
"standard" citation issue; I have also followed Mill's comments on APG. The
most important issue seems to be consistency. Different publications use
different citation standards so one must also be flexible.
