Hello,

I have one for exactly that year. It includes:

Registration District (and sub-district if applicable), County, number of
entry, Where and when died, Name and surname, Sex, Age, Occupation, Cause of
death, Signature, description and residence of informant (that is, the
person who went to the Registrar's office to register the death), Date
registered, and signature of Registrar.
