I think it is definitely MRCS - in the 1861 census he is down as
surgeon ( so Member of the Royal College of Surgeons)

Either Susannah acts as his dispenser and the Licenciate of the
Central Apthecariis Company applies to her - or more likely given the
time and her gender, the bracket means that should apply to Thomas
Leaman as well.
In 1861 Susannah is married with a small baby.
I think the general practitioner actually applies to the first 'medical
assistant' listed in the household, George Slade or Cade
So 38 Church Street appears to have operated as a general practice
surgery and pharmacy.
