When my son was born in 1972, we didn't name him right away, as his father
and I were not in agreement. When I went home from the hospital, we still
had not agreed on a name, and the nurse told us that the Health Dept. would
send us a form to complete since they couldn't complete it there. We agreed
on a name when we got home, and sure enough a few days later the envelope
from the health dept arrived. However, when I opened it, was I ever
surprised. My baby boy had been named after his father, with Jr. appended.
That wasn't even a name we were considering, as neither of us liked his
name. I called the Health Dept, and asked how I could get it changed, and
they said at that point, I would have to file papers and get a court order.
I called the hospital and asked why the nurse did that, and she had no
recollection. The best we can figure was that because we didn't have name
picked out, we were referring to him as "Junior", and so she filled out the
paperwork to reflect that. Well, we were young and broke, so his name
remained the way the nurse named him. As a rather funny aside, my mother
decided that the name didn't fit, so she took the liberty of renaming him
Butch, which I hated even more than his father's given name. Poor kid.

There is a LOT of that going around.
If th parents do not name a baby immediately, the official name gets
recorded like that.
