I think it's $8.00 now. I found this website:

They seem to have a similar service for all the
states. Just substitute the two-letter state
abbreviation for 'ar' in the URL above. Although this
is a dot-com and not a dot-gov, the AR site says to
make checks out to the Arkansas Dept. of Health -- so
I reckon it's on the up-and-up.
