The SS-5 form you can request from the Social Security Administration can be
ordered in 2 ways. An actual copy of the form which costs $27.00 or the
numident printout which I believe is $17.00. The actual copy of the form
will have parent's names, maiden name of a married woman, residence,
birthdate, place of birth, and depending when the form was filled out the
employers name and address. The numident printout does not give you as much
information as the copy. I believe what Ancestry.com has is the SSDI, which
is the Social Security Death Index. This just lists those individuals whose
death was reported to the Social Security Administration - usually by a
funeral home or by the family of the deceased.
