You might find "Indexing Family Histories" a special publication by NGS,
to be helpful. Published in 1994, it is, to my knowledge, the most
recent publication on the subject, especially one published by an
"authority."
A couple of points made in that work are worth mentioning. As others
have already said, index all people in a work by all of their names
known to you, even if that particular name does not appear on the page.
For example, if Mary Jones is shown by her married name of Mary Smith on
page 38, index her on page 38 as Mary Jones and cross reference to that
from Mary (Jones) Smith.
Another point made in this work is to produce only one index. One
rationale for this was recognition that novice researches often read a
book from it back. They turn first to the last index in the work, easily
missing other indexes. With appropriate cross-referencing, a single
index can be more useful than multiple indexes as it helps the user (the
only one we create indexes for) to find more ancestors in the indexed
work.

indexed by only their married name or by maiden and married name?
