Province of British Columbia - Registration of Birth [by parent or
guardian] Place and Registration #/
Place of Birth. City,Street. House number.
Full name of Child.
Sex. Single,Twin,Triplet or other - Was the child born alive. Are
the parents married. Date of Birth.

Section for information about father:
Full name. Residence. Nationality. Place of Origin Age at last birth
-years. Birthplace. Occupation. trade or profession.Business in which employed

Section for Information about mother:
Full name. Residence. Nationality. Racial Oricing. Age at last
birthday.Birthplace.
Children of this mother [including this birth]
a.number born alive. b. Number now living. c. Number stillborn [born
dead after 28 weeks pregnancy]
Was this a premature birth:?

Name and post office address of informant.
Name of doctor,nurse or other person in attendance at birth.
Marginal Entries. * any changes etc - if adoption occurred -
information would be included here - sometimes years later.
Then words to state that this is a sworn statement of truth/belief
Date. Signature of parent
signature etc of Disctict registrar.

**CERTIFICATE: is an Extract of Basic Information taken from the
Registration. Good for proof of an event - A piece of Identification.
